IHaving awoken from their spring break extravaganza at Lake Victoria, the swarm heads upstream where they look to make a meal out of Big Wet, a local water park where when it comes to fun, nobody does it wetter! Though they came to get wet, get loaded and get some, the staff and patrons get more than they bargained for when they must face the fiercest, most bloodthirsty piranhas yet. Lead by the strong-willed, studious Maddy and her friends, Barry and Kyle, the trio must dive in and take on these man-eating creatures using every ounce of their being but can they be stopped?

Nội dung phim là câu chuyện tiếp diễn của Piranha 3D ra mắt năm 2010, nằm trong loạt phim ăn khách về loài cá ăn thịt người hung tợn. Một năm sau vụ thảm sát tại hồ Victoria, đội quân hùng hậu của loài cá tiền sử Piranha tiếp tục "ngao du thiên hạ" qua các đường ống trong lòng thành phố với đích đến là các bể bơi trong công viên nước vừa khai trương "Big Wet". Để tiết kiệm chi phí, người quản lý công viên đã cho dẫn nước từ hồ vào mà không biết rằng họ đã tự mở con đường xuống địa ngục. Từng đàn cá ăn thịt hung dữ theo dòng nước qua các ống dẫn, thỏa sức tàn sát con mồi là các du khách đang vui chơi ở đây.
